Counties Required to Report Customer Satisfaction Data
The following counties are required to submit customer satisfaction data for FY25 (September 1, 2024 – August 31, 2025).
All agents for counties on this list must 1) collect survey data using a designated customer satisfaction scan form or online survey for at least two activities conducted during the FY25 period and 2) submit the completed forms to ODA.
