Create your own test of knowledge using true-false or multiple choice. Simply enter the question text and responses for each question.
Important Notes:
These forms only include test of knowledge questions – nothing else – but you have options on the number of questions to include on the test (see details below).
Step-by-Step Instructions
- Develop your test of knowledge in Word – questions and response options (if multiple-choice) – before using the scan form.
- Cut and paste or type in enter your questions and response options (if multiple choice) into the form. Print two copies.
- In the “Office Use Only” box, fill in the pre-bubble on one form; fill-in the post-bubble on the other form.
- Hide any unused questions by simply cutting and taping a piece of white paper over them. Be careful not to cover the survey id or cornerstones.
- Make copies of both the pre and post tests (copy two-page forms on single sheet).
- Send a list of correct answers with your completed forms and cover sheet.
